Healthcare Insurance Data Cleanup

I need an experienced Excel professional to take a raw set of healthcare insurance-claim records and turn it into a clean, analysis-ready workbook. The file currently includes inconsistent date formats, duplicated rows, and non-standardised provider and procedure codes; my priority is to have every field validated, re-formatted, and organised so downstream reporting is effortless.
Scope
• Thoroughly clean and format all columns (dates, numerical amounts, text fields, ICD/CPT codes).
• Identify and remove duplicates while preserving the first complete record.
• Flag obvious data entry errors or outliers in a separate tab.
• Return a single Excel workbook with a tidy main sheet and a brief change-log.
Acceptance criteria
– No duplicate claim IDs remain.
– Dates follow a uniform DD-MM-YYYY structure.
– Currency figures are numeric, two-decimal, and free of symbols.
– Code columns match the official ICD-10 and CPT patterns.
– Workbook opens with no validation errors.
