Excel 4M Analysis Template

I need an Excel workbook that lets my team record every step of a 4M (Man, Machine, Material, Method) investigation and immediately transform that data into highly detailed, category-specific reports.

Core data entry has to cover:
• Vehicle / VIN, Area, and free-text Problem Description
• A fishbone-style section for selecting or typing causes under each of the four Ms
• A simple checklist confirming which 4M items were reviewed
• Root-Cause Summary, Proposed Action, Person Responsible, Target / Completion Date
• Space to embed or link Photos and other attachments

Once the information is in, I want a “Generate Report” button (or a clear instruction) that assembles a polished, drill-down report showing the findings and metrics for each 4M category—think pivot tables, slicers, or Power Query—so we can filter by vehicle, area, or date and still keep the narrative intact. Visual elements like conditional-format heatmaps or compact fishbone charts are welcome as long as they feed the customized reporting focus.

You’re free to use formulas, pivot charts, VBA, or Power BI links; what matters is that someone with basic Excel skills can start entering data on day one and click once to get the detailed analysis we need. Keep the file lightweight, clearly labeled, and ready for me to test with sample data.
