I need a licensed or clinical pharmacist to analyze anonymized hospital patient data. The goal is to assess drug therapy, medication effectiveness, and drug interactions, and propose treatment optimizations.

The data is in Excel and includes:
- Lab results
- Medications
- Diagnoses
- Clinical notes

The report must adhere to our hospital's internal clinical pharmacy guidelines. Knowledge of pharmacokinetics/pharmacodynamics is preferred.
