Carehome intake agent

Hi! I want to build a care intake agent. Example... I have a homecare agency, where we go to people's house and help them feel better. Mostly people call for their very old parents or grand parents. When they contact, we need to ask them many questions and it takes up too much time on the phone. Last night I went into Zapier and created the form.
The problem is, I cannot get it to communicate correctly. I have the proper questions and automation I need but I am doing something wrong. Ultimately, once the form is 100% ready with all automations, how do I install into my website? That part I don't understand very well either.
If I want to share with my 3 friends that also have agencies, we all send each other work, how do I share it with them so they can use it in their website as well? The goal is to create a great one for myself, install it on my website, then make a great generic one that I can share. Are you able to help me with this??? What will you charge?
