Web Application Interface Design & HTML

We are looking for a skilled designer to create the interface design for a screen of our web-based application. The design should be visually appealing and user-friendly for both web desktop and tablet/mobile devices. The deliverables will include two jpegs - 1 for web desktop and 1 for tablet/mobile view, as well as responsive HTML/CSS code. The ideal candidate should have experience in UI/UX design and proficiency in front-end development for web-based apps. Attention to detail and ability to understand and incorporate client requirements are essential. Good communication skills and the ability to work collaboratively with our team are also important.

Attached is a PDF file with a mockup of the functionality/content we will need - all of this content & text is fine to incorporate into your interface. Also attached is a jpg for style direction (colors, font, etc.) however we are open to hearing other ideas.
