Template web (HTML5+CSS only) for warehouse
Budget: €30 – €250 EUR
I'm looking for a freelancer to create a new form template within a timeline of 2/3 days (test included).
Informations are sufficient to provide an estimate (times and costs).
I hate "placeholders": put the final offer (money needed by you).
Only HTML5 and CSS. Javascript/jQuery only if really needed.
Using free objects/sources freely available on the internet is really appreciated (and reduces testing and development time).

The template has ONE FORM only.
During our job activities, some BU (offices) need to require something from an office to another.
The new form must contains all fields that we need to fill (see attachment).
At the end of the form, before confirmation, documents upload (if any).
Login form not necessary. Only one (vertical scrollable or horizzontal with mulpitle DIV) form.
Browsers in use are Firefox and Chrome.
Template will be tested and used into an Intranet.

Pay attention to quality of details.
No upfront. No payment before successful completion of all tests.
Unnecessary images, files, libraries, ... must be removed.
Source-code required for test. The source code is required to have a good level of comments, to allow bug-fixing, code maintenance and possibly future evolution.
Please, no ask me «what is your budget for this project?»

Proposals from programmers who do several activities in parallel will NOT be accepted: the quality of work could be low.
Start as soon as possible - daily update is requested (showing progress), to avoid time loss and misunderstandings about requirements.
Without daily update with project progress, project will be un-assigned.
