Single page form

Project Title: Single page survey form

Requirements:
- I need a single page survey form designed and developed.
- The form should have 1-5 fields for collecting user input. The fields will be images as well: something like https://madmuscles.com/step-goal
- The main purpose of the survey form is to collect email subscriptions.
- The ideal candidate should have experience in designing and developing survey forms.
- Knowledge of HTML, CSS, and JavaScript is required.
- The form should be visually appealing and user-friendly.
- It should be responsive and work well on different devices.
- The form should have validation to ensure that the user enters a valid email address.
- The collected email subscriptions should be stored in a database or sent to an email address.
- On form submission the user should get an email with a PDF based on the selection made in the survey.
- The project should be completed within a specified timeframe.

We only need source code with steps to set it up and deploy on Cloud (AWS/Azure, etc)
