Shopify Form & UI Refresh

I want to give our Shopify store a quick but meaningful upgrade. We have already purchase an ELLA Shopify template but need some to revise the layout to meet our needs. The top priority is two embedded order forms that lets shoppers input data in about 7 fields on two different forms before adding the item to cart or checking out. It needs to feel native to our theme and work flawlessly on mobile.

I have one of my team completing artwork so there is no design artwork needed. I have already completed a file that outlines the layout and we have logo/brand colour palette to use on the page so I believe the project will be straight forward for a confident coder.

Project needs to be completed within 7 days

Deliverables
• Custom Liquid/HTML order form with product selection, quantity selector, and notes field, fully tested on desktop and mobile
• Site-wide color scheme update (variables, CSS, theme settings)
• Redesigned homepage layout implemented within the current theme, preserving SEO structure and existing content blocks
• Brief hand-off doc or Loom walkthrough so my team can adjust form fields or colors later
