Search Form - Bootstrap + HTML + CSS

Hello!

I need a form created that uses Bootstrap version 4.6:

I will need to see the form display as expected, and will need the HTML and CSS provided to me.
I will expect the form to use the input-group controls provided by Bootstrap:
