Responsive HTML5 Form

I am looking to commission a responsive Mobile/Table/PC HTML5 form

Main requeriments
• Not interested on automatic / generic bids
• Read the document before you bid
• Vanilla javascript, not framework
• You can use a CSS template but as light as possible
• One page / not reload or multiple pages
• Mobile / Tablet first, extremely care about this
• As easy as possible to fill in mobiles devices.
• Webcam Image capture
• Signature pad
• Multilanguage

Document it's pretty clear and covers every aspect of the logic. may seem heavy but it is quite simple and straightforward
