Municipal Motorcycle Officers of California (MMOC) Apparel Order Form

I have an order form that was built in Adobe Acrobat Pro. It already lets shoppers tick the items they want, tallies each line, adds shipping, and shows the grand total automatically. The catch is that most recipients only have the free Acrobat Reader—or they open email on a phone—so they lose the smart features.

Your task is to rebuild or convert the form so that I can simply attach it to an email and every user, whether on a desktop, laptop, tablet, or smartphone, can still:

• check the boxes they need
• watch the totals update instantly
• either submit the completed form back to me electronically or print it out if they prefer

I’m flexible on the underlying format—PDF, Excel, or a lightweight HTML/JavaScript form are all fine as long as the calculations remain intact and no paid software is required on the user’s side.

Deliverables:
1. The finished, self-contained form ready to email.
2. A short note explaining how I can update prices or product lines later without starting from scratch.
3. Any simple instructions the end user might need (for instance, “click Submit to email” or “tap Print if you prefer a hard copy”).
