Modern PHP Quiz Web App

I want a lightweight online quiz platform built with PHP, MySQL, HTML / CSS and vanilla JavaScript. The interface should feel modern and sleek, loading quickly on both desktop and mobile.
Core flow
• Visitors create an account, confirm it with a straightforward two-factor step, and then log in.
• Once inside, they can start a timed multiple-choice quiz. Questions appear one at a time; when users submit an answer they immediately see a short explanation, and at the end the system totals their score and stores it alongside their profile.
Admin corner
An authenticated admin dashboard lets me add new quizzes, edit or delete questions, and view aggregated results. Everything should be intuitive enough to hand over to non-technical staff.
Data layer
Use MySQL for users, questions, answer explanations, scores and basic audit logs. Please keep the schema clear and include an SQL dump for easy setup.
Deliverables
• Full source code, clearly commented
• Responsive UI matching the modern aesthetic described
• Database schema and install script
• Brief deployment read-me (Apache or Nginx)
• One short hand-off session or document so I can extend the question bank myself
Acceptance criteria
1. Sign-up, 2FA, login, password reset and logout all function without errors.
2. Admin can create, update and delete quizzes and questions from the dashboard.
3. Users can complete a timed quiz, view in-line explanations, and receive an accurate final score.
4. Results persist in the database and can be exported via the admin panel.
5. Code runs on a standard LAMP stack with no proprietary dependencies.
