Fix CSS issues in the simple form based on the custom Bootstrap 4

Attached are two folders with the same form, but different CSS and JS.
The one in "working" folder is working well, with all form checks and error highlights.
I need your help to modify CSS/JS in the "to-fix" folder to work exactly as in "working" folder.
Your fix should introduce the minimal (only necessary) changes into CSS/JS in the "to-fix" folder, because these CSS/JS are used in the rest of project and they also should be minimal of size.
