Editable Three-Page Word Application Form

I need a clean, three-page application form built in Microsoft Word (.docx). The finished file must be fully editable so my team can update text in the future, and it has to include working tick-box fields (Word developer controls) wherever a yes/no or multi-choice response is expected and end user can supply content.

Content scope
• Purpose: this is an Application form, not a survey or feedback sheet.
• Core sections: Personal Information and Health Information are mandatory. We will supply all the necessary information
• Logos: I will provide these; please place them in the header so they repeat automatically across all three pages. No other graphics are required.
• Style: plain, professional, black text on white background, clear paragraphs, section breaks using standard fonts available on every Windows installation (Calibri or Arial work fine).
• Layout: balanced white space, left-aligned labels with form fields or checkboxes flush to the right for easy data entry. Pages must have identical margins and a clear footer for version/date.

Deliverables
1. A three-page .docx file with active tick boxes and editable fields.
2. A PDF export for quick review of the layout.

Acceptance criteria
• Opens without macro warnings in Word 2016+.
• Checkboxes are clickable and stay aligned when text is edited.
• No text, table, or header shifts if I delete or add a line of instructions.
