Custom Form Builder Web App
Budget: $750 – $1,500 AUD
I need a web application that lets my users build their own Likert-questionnaire forms, send those forms to their clients, and then view and compare the completed responses in clear summary reports.
Core workflow
• User logs in, chooses a customizable template, and selects or edits questions to create a Likert scale form.
• The finished form generates a unique link that the user can email or share with a specific client.
• The client opens the link, completes the questionnaire, and submits.
• All responses are stored securely and tied to that client record.
• Inside the dashboard, the user can open a “Reports” tab to see aggregated results and key statistics for one client at a time, with options to export or print the summary.
Key requirements
- Form builder with Likert scale question type as the default, plus the option to add additional question text or response labels.
- Template system: pre-made layouts the user can customize (which questions, order of questions).
- Secure user, client, and role management.
- Persistent data storage (SQL or NoSQL is fine as long as it supports future scaling).
- Summary report generator that surfaces averages, top-line insights, and response counts.
- Clean, responsive UI (React, Vue, or similar modern framework preferred; open to suggestions).
- Well-structured backend (Node, Django, Laravel, or comparable) with REST or GraphQL endpoints.
- Deployment script or Dockerfile plus short documentation so I can run the app on my own server.
