I need someone to redesign this HTML form to a mobile-friendly format:

For example, redo the entire form in Bootstrap or even yet, something more lightweight.
All the functionality and fields must remain the same (same names, same values, calculator still working...) just redesign and restyle it to make it mobile-friendly.
You will NOT be doing any back-end coding, only the front-end needs to be modified.
Need this done within 24 hours of bid acceptance!
