Automate Mailchimp and customize google form -- 2

We have a list of 600 clients in Mailchimp
We would like to send an email to each one with a specific link that leads them to a satisfaction survey ( google form )
that google form would collect a hidden var passed in the url ( the email or ID of the client )
Hence the customer will answer openly and "anonymously"
the point is to collect the answers and know who answers what
Also, with conditional logic, if the customer answers positively, automatically, send to him an email with an invitation to evaluate us in google business and facebook
We planned to do this in google form,
but any other option is ok
