Add 2 Landing Pages on WordPress (Form + Data Display) - 30/09/2025 18:21 EDT

I need two new landing pages added to my existing WordPress site. Work must be delivered As soon as possible .

Requirements

Login System

Both pages must be login-protected.

Login: admin / admin123.

Landing Page 1 (Form Page)
Form fields required:

Mr./Ms. (dropdown)

Last Name, First Name

Date of Birth

City, Province/State, Street & Number

Desired Profession, Desired Location

Apprentice (Yes/No), Diploma (Yes/No), KMK Evaluation (Yes/No)

Language Level (A1–C2 dropdown)

Language Certificate Date

Commission/Fee

Document Upload (PDF/DOC/DOCX)

Landing Page 2 (Data Display Page)

Shows submitted form records in sequence (with numbering).

Supports pagination (10–20 per page).

Uploaded documents must be downloadable.

General

Unlimited submissions supported.

Simple, clean, professional design (not plain white).

Must integrate properly into my WordPress site.

Responsive (desktop + mobile).

Deliverables

Working demo first (can be on your hosting).

After approval, implement directly on my WordPress website (login will be provided).

Award will be given as soon as the demo works correctly.
