Fix cPanel MySQL Database Mapping Issue (DBCACHE exit 255)

I am looking for an experienced Linux / cPanel & WHM administrator to troubleshoot and fix a database mapping issue on my self-managed VPS server.
Server Details:
VPS server (root access available)
cPanel & WHM version: 11.134
MariaDB running normally
Websites are working and databases are functional
Problem:
In cPanel → Manage My Databases it shows:
"The database server is currently offline (adminbin Cpanel/cpmysql/DBCACHE: exit 255)"
phpMyAdmin was also inaccessible earlier.
Databases and MySQL users exist and websites connect successfully.
After rebuilding DB mapping some databases appear but:
Database privileged users are not showing in cPanel UI.
cPanel database mapping seems broken.
Troubleshooting Already Done:
Verified MariaDB service is running.
Tested database connectivity via MySQL CLI.
Ran update_db_cache and rebuild_dbmap scripts.
Attempted dbmaptool mapping.
Restarted MySQL and cPanel services.
Ran cPanel update/repair commands.
Checked /var/cpanel/databases mapping files.
Current Situation:
Websites are live and working.
Databases exist in MySQL.
cPanel UI does not properly display database users and privileges.
Need proper repair of cPanel MySQL mapping / DBCACHE issue.
What I Need:
Diagnose the root cause of the cPanel MySQL mapping issue.
Restore proper database-user mapping in cPanel.
Ensure phpMyAdmin and Manage My Databases work normally.
Verify that databases, users, and privileges are correctly synchronized.
