Type and make a PDF document -- 2

I am looking for a freelancer to type and create a PDF document for me. I have scanned my own handwritten notes and need them in typed form.

Specific Requirements:
- Convert handwritten notes into a typed PDF document
- Follow the specific format that I have in mind
- The document has more than 20 pages
