Perfect PDF to Google Docs Conversion

We need a PDF converted to Google Doc. The PDF has many gridlines, checkboxes and layout details that do not easily conform to the simple feature in google called "OPEN WITH" which is what we need someone with experience. We need to perfectly duplicate this PDF into the Google Doc format , this means duplicate the content and layout and details of the PDF
