Trucking Employee Contract

I need to write up a employee contract that is legally binding that protects our drivers and our business.
I would like it to include things like;
1.Agree to adhere to all company policies laid out in Employee handbook, with the date (month/day/year) and spots for our names and the employee's name, as well as in what state
2. Verify that all the info about themselves and their credentials/ certifications and or endorsements (med card/CDL) are true and valid/uptodate.
3. Time and Attendance - expect them to be on time, communicate
4. Do their job to the best of their abilities
5. Damages/Liabilities - Report any damage to truck immediately, who is liable..us UNLESS they didn't follow all safety checks or were impaired by drugs or drinking...
6. Safety and Security - agrees to perform all safety checks EVERY drive, as outlined by State laws, and never drive in unsafe conditions....either with themselves or with the truck, just to get money or get the job done.
7. Termination - not uphold their end of the contract by doing their job to the best of their abilities or found out to be lying about loads, in the probationary period it will be immediate termination, but afterwards we would give them 2 weeks? They must give us 2 weeks notice if leaving of their own accord. Must return any and all company property...work phone, work tablet, hard hat, etc...
8. Dresscode - agrees to abide by all state safety dress codes
9. Clients - agrees to deal with all clients with respect, do not argue, tell us and we will deal with it. We will support you and back you up, as long as you have done your part to stay respectful/honest.


Looking over this I will try and write up something that states what they can expect from us, and what we are expecting from them...
Really, honesty and hard work are the biggest things for us. We want to see them succeed. We want them to do well. We want their job to be easy, like we will support them however/as best we can, by making sure they have what they need to do their job, as well as helping them to get the time they need for their families. In return we ask that they work hard for us. Do a good job, like represent us well.
