Pooltexx Service Contract

We are adding a new service to our company and it requires a contract. We are a swimming pool service & repair company. We do weekly service and equipment repair only. No structural or construction type work on the pools. We are going to be offering a contract to our customers to do certain repairs to their equipment in case of a breakdown for a monthly fee. We are looking for a contract that includes our terms, conditions, pricing etc.
