Onlyfans model contact

I own an onlyfans modeling agency and need a written primary contract. I need confirmation that the model has agreed and provided evidence that she is over 18 as well as an agreement to post her pictures online. I'll also be needing an agreement of payment for 30% of total profits made from the only fans account. Contract length to be 3 months.
