Create Non Disclosure Agreement/Contract - Full Disclosure

Non Disclosure Agreement/Contract - I think this should be pretty easy but I need someone to write this up who has a bit more experience in this side of the business than I do who is used to these sort of things. Dont worry I'm not asking for a lawyer here, just someone who can articulate well and within sound logic the following contract NDA...

I need someone to create an agreement between 2 parties that will do the following:
All of the regular things such as keep all information learned within the project about the project confidential, a gag order basically of talking about and/or sharing with any other parties any ideas, concepts, artwork, designs, media content, music, literature, documents, etc... Don't give away secrets to anyone. Don't use the proprietary technology and/or information for any other personal use, or business reasons for now while working in this project or even after any sort of termination for up to 5 years.
More importantly I want part of the disclosure agreements to have all parties LIST ALL projects and work they are currently doing and plan on doing now or in the future that has or can have any impact or affect with the current project. I want listed all projects the person is involved with now and in future plans that have or could have anything to do or could possibly have any impact or conflict of interest. I want all projects and potential conflict of interests fully 100% disclosed. And I want to know now and in the future if anything like this arises.
