Individual Service Plan Report

A resident has just completed a full Department of Social Services assessment and I now need that data transformed into a clear, actionable Individual Service Plan. The finished document will be read primarily by our healthcare providers, so the language and structure must align with clinical standards while remaining accessible to nurses, therapists, and case managers alike.
Focus areas
The plan should concentrate on the resident’s Medical needs and Social needs—detailing goals, interventions, responsible parties, and measurable time-frames for each. Educational considerations or other domains can be noted only if they directly support these two core areas.
Format
I need the ISP delivered as a narrative report (Word or PDF). Charts or tables are welcome inside the report if they clarify timelines, medication schedules, or social-integration milestones, but no separate slide deck or spreadsheet is required.
Key content I expect to see
• Resident profile and summary of the DSS assessment findings
• Goal statements for both medical and social domains, framed in SMART language
• Specific interventions with frequency, duration, and the professional responsible
• Coordination notes that show how medical tasks interface with social supports (e.g., community outings, family involvement)
• Review schedule and criteria for adjusting the plan
