Scope of work
Please create a separate JSA for each of the following activities:
• Excavation and trenching
• Working at height
• Heavy machinery operation
• Hot work
• Cold work
• Confined space entry
• Lifting and shifting
• Fire-related tasks
• Chemical-related tasks
• Manufacturing-related hazards that may overlap the site
What each JSA must contain
1. Step-by-step task breakdown
2. Hazard identification and initial risk rating
3. Practical control measures aligned with the hierarchy of controls
4. Residual risk rating after controls
5. Required PPE and any permit references (e.g., hot-work, confined-space)
6. Sign-off fields for supervisor and crew
