Jotform Specialist for Medical Practice
Budget: $30 – $250 AUD
We are seeking an experienced freelancer to assist in creating an online form using Jotform for a medical practice.
Requirements:
- Strong experience in Jotform form creation, including conditional logic, calculated fields, hidden sections, and automated workflows.
- Solid understanding of medical terminology and healthcare data capture (e.g. patient intake forms, risk assessments, health screening questionnaires).
- Ability to design user-friendly, compliant, and visually consistent forms suitable for both patients and clinicians.
- Experience configuring email notifications, PDF reports, and integration with practice systems (e.g. Google Drive, email, or EHR platforms) is highly regarded.
