I need a petition builder
Budget: $250 – $750 USD
... for a petition I've written that has six fields to fill in,
1. name,
2. their zip+4 (with link to U.S.Postal Service for those who don't know it) and with short sentence next to it
3. their state (automatically filled in based on their zip)
4. their country (a drop-down)
5. whether they have children in their lives they care about Yes/No
6. whether they're a registered voter also a Yes/No, with short sentence next to it
Petition will also show how many others have filled in that same zip+4, state and country, so it needs to showing running totals. Also need this fairly quickly.
