Creating School Selection tool for website

I want to get a tool created, which would be embedded in our website, which would help prospective candidates know which B-schools they can target. It'd be a form with around 12 questions and around 4-5 options for each question, which the candidate would choose. I've an excel sheet where a score for each option is mentioned. The tool would calculate the total score of the candidate based on the options chosen by a candidate. I've created 6 score brackets. The tool would then apply few filters in a school excel sheet depending on the preferences chosen by the candidate and score bracket the total score would lie in. This would return few B-schools, which will be categorised into dream, reach and safe school categories depending on the total score bracket. The output would be a pdf report with the schools mentioned in it, which will be downloaded on the user's system and emailed to the user.
