Build me a Simple code for algorithm with few entries -- 2

I want CVD risk prediction using WHO 2019 charts, where I want if i put entries as Male/Female, Smoker/Non Smoker , Age, BMI and Systolic Blood pressure, it should show the risk using chart attached, with color coding also, simple Project, I will also pay if asked for reasonable amount.
