registration form dosnt work (JS, PHP, MySQL) -- 2

1) i cant register any new accounts, pls check and fix where problem
i see new email address have added in db table but any email come with confirmation and any message on website like account is created successful
a) make validation for email address when is exist show message address exist (i have seen doubled addresses email i db table)
b) add in table with users account (table name: uzytkownicy? ) info about registration date
c) message after account is registered

2) after register new account, i want send email (thx for registration bla bla bla, that work before have to check)
3) add to field password (eye icon, when click i can see password)
4) i need to add text under buttons google, fb (restore password - link same as in login page), after click to reset link i can put adres email and script will send new randomly generated password

another TO DO
1) phpmyadmin databse run very slow, some times have to wait long time, check where problem
