survey after interview

I want to have an automatic survey sent to the candidates after each interview. can you offer something that I can purchase for my company to set up our questions and have them automatically send it when their interview is complteted?
I would like it to be set up that it says who their Interview was with. for example I hope you had a nice interview with Debbie...
