Concrete Footing Design for Pipe and Pedestal Kit

I need a concrete footing to be designed.

Approximate dimensions: 4' x 4' x 4'.
Soil: 3000 psi
Steel: metal cage 5/8" rebars @ 24" o.c.

The concrete footing is intended for a pole support for a satellite dish.
