Stylized Werebear DnD Character Sheet

I'm looking to have a stylized DnD character sheet created for a Werebear. The character sheet should have a dark and brooding to include light armor (shoulders/pauldron, chest/cuirass, knees/poleyn) , battle axe (either in hands or across back, character looking up like its howling, male character, incorporating elements of armor and weapons, as well as background scenery paw prints, trees etc. With boxes on sheet for character, name, abilities, attributes, based on druid dnd character.
