Recreate an Online Form as a Document
Budget: $30 – $250 USD
I want to create an A4 Document out of this test & waiver form at https://form.123formbuilder.com/6356165// It runs to about 10 pages.
The end product would be a hard copy waiver that I could use instead of the online waiver and I expect this would probably need 4 sides of A4 to accommodate all the content.
I would expect the test questions to be nicely laid out with maybe 4 questions on each page. A checkbox for customers to complete the test.
Feel free to move around the content. So, for example it makes sense to have the personal details upfront instead of the test upfront.
It needs a title like "Knowledge Test & Waiver"
I have attached our brand fonts, although we don't use pink! The business is Quad Bike King and you can see us at https://quadbikeking.com.au.
Finally, we're always editing the terms in this waiver and I also want to do it in multiple languages, so I will need access to the original file. I don't have photoshop, so I'm imagining me giving you shared access to a Canva document might be a good way forward.
Of course, I will be flooded with bot and chatgtp responses, so please try and differentiate yourself from that crowd to get my attention by saying something early on that does so
