Generate a Bas Relief portrait for CNC from draw

For this portrait I need a 2.5D bas relief model for CNC machining. You will find the portrait joined. No automatically generated ones. Have three other portraits to generate just after this one.
Definition should allow CNC for surface of 1m length and 40 cm large.
Depth Max of machining 20mm on MDF.
