URGENT! Functional C# App (easy)

I have a couple Windows Forms (.NET) data entry forms, one of which is the main menu (I have attached the picture below) and I would like to make a complete application that will open on that menu, and when you click on any of the buttons in it to open the corresponding submenu (opening another form and closing the current one). I have a database in MySQL where the data entered in Windows Forms will be sent, I have ALL the Windows Forms made, I basically need someone to link the forms together. More details on chat. It is urgent, I need the application made today.
