Small C++ form with radio buttons wanted

I'd like a small C++ form provided which displays a
list of cam devices detected (I can provide the C++
code for getting the list of cam devices). Attached
CamListForm.png shows what app must present.

1 or 2 cams are typically detected, notice radio
button to right and notice the OK and Exit buttons.

My budget to have this cpp app provided is $30 max.
I need to be able to build it with VS2017 or VS2019.

If you've developed something similar to this
before please show me a picture of it.
