Small Visual Studio Project

Seeking a knowledgeable C# programmer to complete a small yet interesting project in visual studio.

Deliverables

A Microsoft windows installer comprised of the following screens:

- an attractive splash screen with logo (supplied)
- form 1: a user profile page
- form 2: a one question questionnaire (3 tickboxes)
- form 3: installation directory
- forms 4,5,6: questionnaires with 6 fields each and a submission button

* Please note: only one form from 4,5,6 will be completed, based on the answer given in form 2. The other two should be hidden if not required.

Data from the above fields to be written to a .json file in the installation directory

That is all. No other functionality or features required. This is a simple data entry and submission exercise.

Requirements:

- clean, commented code
- full project source code and dependencies (ie not just a compiled executable)
- timely delivery
- freelancer must sign the freelancer.com IP agreement
