SQL Report on Customer Data - need to be in 24hrs

Need to be in 24hrs- I have data across 4 tables in MSSQL and need to standalone Windows EXE that will generated one(1) single report. Initial screen is a form to select some filters/options for report - date range etc and then run the report. Need display the result and save the result as PDF. 2nd part is need a Config Screen that allows user to enter MSSQL address, DB name and U/N and Password for (SQL Server AUTH) OR Windows AUTHENTICATION (like in SSMS). I have a fully functional online PHP demo and can give access to the php code. I also, the sample of the DB Configure screen .
