Need to create a dental tool in .net desktop application

I have a clinical software for a dental practice. They need to add a new feature where the doctor will make diagnosis of the patient using a graphical representation of the teeths. If he selected the tooth one time it will change state and color of this tooth and he can write information for this selected tooth. You must record the information in the database.
