EHR Form Builder

We would like to have an option for our clients to create and use their own forms in our electronic health records software. We have 35 placeholders built-in that we would like to have the option to include. We also have an eSignature option built-in. We would like clients to create their own Intake Forms that they can then have their clients complete and submit electronically.
