Desktop Application to autowrite text reports - C# .NET
Budget: $15 – $25 USD
Program that writes reports primarily with clickable content, which is highly editable for my uses. I am a Chiropractor and I write a variety of reports. Typically, the content is repetitive, with minor alterations. If I have written the content already into the program, then I can click through the data I want to ad, a fully fleshed out paragraph form report will populate on a template, in real speech, which makes sense. I would have to provide all the specifics as to what is written on the backend. This should also be editable, so if I can design my own new templates, and inputs on the back end without requiring new programing.
So the first thing to do would be to create a patient information screen, where I would enter items like, First name, Last name, age, date of accident, date of birth, email, phone number, address, sex, title (Mrs., Mr., Dr., etc.), file number. This would allow a database of existing patients to be present, so this wouldn’t need to be entered for each patient. Data from this would be populated into the report, for example- “Mrs. Jones performed cervical range of motion assessment of the cervical spine.” Or “She/He” etc. On report templates, the patient’s age, and gender would autopopulate.
The next would be the type of report, which would then lead to a column of choices on the left, with each being selected, could generate sub inputs.
There should be an option to save ones work through the process, or even better, it autosaves between input pages.
I have seen a very similar design in an existing electronic health record system. I am looking to have this expanded and more robust, and versatile. Please see the links below:

At the end of the process, it would give me an option to open the report in Microsoft Word, or simply populate it to the clipboard for pasting into my electronic health record system. It would also save locally automatically to the patient’s name.
