I'm looking for an Excel spreadsheet to track incidents and accidents at the workplace. Must conform to ISO 9001:2015 format requirements

Requirements:
- The spreadsheet should capture specific data points for each incident, including:
- Date and time
- Location
- Type of injury
- Equipment involved
- Specific location of the incident

- A dashboard within the spreadsheet that visualizes the data using pie charts.

Ideal Skills:
