Jotform Expert Needed only from Pakistan

I would like someone to create or basically convert the form of the attached doc to an Online form using Jotform.

I would like users to fill up the form online, however, once the user submits the form, they will have a preview PDF form and once they click on that button all the responses will get auto-filled in the original pdf and they can download it.
