Build me a Bubble.io Web Application

I have an existing prototype that acts as an online dietician. The product works like this.

Website --> questionnaire --> results of the questionnaire get fed into algorithm --> algorithm spits out recommended nutritional supplements (vitamins, minerals, super foods) --> the user then decides if they like the recommendation or want to alter it, then they can order the product --> orders get sent to a manufacturer in South Africa (we are South African) --> the product gets manufactured and delivered to the user's door.

We are looking to create a more sophisticated version of our product, integrating our backend with your frontend. We need a frontend to be designed and developed to accommodate our questionnaire and then connect our backend to the Bubble.io API.
