Put content on website and have a form for collecting details

I am looking for a freelancer who can assist me in putting content on an independent website. The content would include converting the pdf brochure into an image content displayed on the website with underneath that image have a link included for opening up an another brochure. Additionally, I need a custom form created to collect contact details from users. The ideal candidate would have experience with website design and development, as well as form building. The form should collect contact details.
