Mechanical Workshop Inspection Report Template

I'm looking for a Word document template for a service inspection report for our mechanical workshop. The template needs to include our logo, with designated boxes for inserting photographs and text boxes for technicians to make notations of their observations. Similar to the attached, however we don't want in PPT.
The report should be structured yet flexible. The report should also have space for general observations
Ideal skills for this job include:
