Design a Daily Diabetes Log Template

Hi, I'm looking for someone to design a user-friendly daily log sheet to help track diabetes control. The design should be in a horizontal layout with fonts oriented along the longer side (9-inch) and could be on an 8x10 or 7x9 page, whichever offers the best balance between size and usability. At the top, there should be a space to record the basal rate (long-acting insulin taken at night), along with designated areas for breakfast, lunch, dinner, and snacks. The log should also include sections to record carbs, insulin dosage, and the time for each entry, making sure it’s easy to read and fill in. I can send an example to work with if needed.
